Layali Medical Supply ("Layali," "we," "us," or "our") is a business-to-business distributor of medical consumables and hemodialysis supplies based in Dolores, Quezon, Philippines. This notice explains what personal information we collect through the Request a Quote form on this website, why we collect it, how it is processed, and the rights available to you under the Philippine Data Privacy Act of 2012 (Republic Act No. 10173) and related National Privacy Commission guidance.

Information We Collect

When you submit a Request a Quote inquiry, we collect the information you provide in that form: your first and last name, your clinic, hospital, or organization name, your contact number, your email address, the product category you select, and the message or requirements you describe.

Please do not include patient medical records, diagnoses, prescriptions, or other sensitive personal information in your inquiry message. This form is intended for product and supply inquiries only.

Separately from the inquiry form, this website uses Google Analytics to understand general usage patterns, such as which pages are visited. Google Analytics may process standard technical information about your browser and device; it operates independently of the information you submit through the inquiry form.

Why We Use Your Information

We use the information submitted through the Request a Quote form to:

• respond to your product or supply inquiry;
• prepare and follow up on quotation requests;
• communicate with you about the products or supply requirements you described; and
• maintain reasonable inquiry and business records where necessary.

We do not use this information for advertising, marketing profiling, or automated decision-making.

How Your Inquiry Is Processed

When you submit the Request a Quote form, your inquiry is sent by email to our team using EmailJS, an email-delivery service, so that we can respond to you directly. Relevant inquiry details are also recorded in Layali's internal customer relationship management (CRM) system so our team can track and follow up on the inquiry. The CRM system does not currently receive the email address you provide — only EmailJS does.

We do not sell your information or use it for any purpose beyond responding to and managing your inquiry.

Sharing / Service Providers

We do not sell or rent your personal information to third parties for their own marketing purposes. To operate this website and respond to inquiries, we work with a small number of service providers who process information as part of the website's normal operation:

• EmailJS, to deliver inquiry emails to our team;
• our internal CRM system, to record and track inquiries (excluding your email address);
• Google Analytics, for general website usage insights; and
• standard hosting and content-delivery infrastructure, to serve the site's pages, fonts, styling, and other technical resources.

These providers process information only as necessary to provide these services.

Retention

We do not currently have a formalized, published data-retention schedule for website inquiries. As a general practice, we retain inquiry information only for as long as reasonably necessary to respond to your inquiry, maintain related business records, and meet any applicable legal or regulatory obligations.

Your Privacy Rights

Under the Data Privacy Act of 2012 and its implementing rules, you have rights regarding your personal information, which may include the right to:

• be informed that your personal information is being processed;
• access the personal information we hold about you;
• request correction of inaccurate or outdated information;

Exercising some of these rights may be limited where we have a legitimate business or legal reason to retain certain information, such as an active quotation or business record.
